Completing Your Three Year Annual PD Goals
Before you Get Started:
These Words are often inter-changed with each other and can be confusing so please note
Non-Content = District or Building
Content = Certification Area = Subject Area/Field Specialization = Personal

SAVE AS DRAFT If you do not complete in one sitting or you lose your work.
Do Not Close Computer or Form without choosing SAVE AS DRAFT or you will lose your work
Choose SUBMIT when completed. You must submit the form to route it to your administrator
STEP ONE:
Click on My Portfolio under My Info
View Licensure
Write down License Date Range: ______________ to ______________
STEP TWO:
Go Back to Home Page by Clicking on My Learning Plan tab
Choose the Correct Form on Left Navigation Bar:
Indiv. PD Plan (Faculty) or Indiv PD Plan (Para)

Your Professional Information
Plan Year:
If the range ends in 2012, this is your 3rd year
If the range ends in 2013, this is your 2nd year
If the range ends in 2014, this is your 1st year
Plan Option: Make a choice. The most common choice is #3
Total CEU Hours for the Three Years
Para Educators = 50 hours (total of both non-content and content hours)
Teachers = 45 Non-Content Hours plus 30 Content Hours for each certification
Date of Submission
Complete with the Current Date ie: 9/20/2011

Non-Content Goals
Overview: Choose Two Building or District Goals and compose a goal for each.
Do NOT use Personal Goals
Building Goals are at the end of the District Goals and are noted by the title: MA Goals or MCS Goals
Select Two District and/or Building Goals
Complete the information for each goal in the boxes underneath
- Goal: Write the Title of the District or Building Goal you chose in this box
- Activity: Write the Type of Activities that you will use to meet this goal.
Click on Link in Form to see Activity Types
- Description: Describe the outcome of what you hope to accomplish in reference to this goal.

Content/Certification Area
Overview: Compose One Goal for each Certification Area you intend to renew
Select box for each Certification you plan to renew
For each Subject Area/Content/Certification Area:
- Code: Copy your Certification information into this box
- District Objectives: select the objectives. More information under section heading.
- Personal Goal: Write the a Title for your goal in this box
- Activity: Write the Type of Activities that you will use to meet this goal.
Click on Link in Form to see Activity Types
- Description: Describe the outcome of what you hope to accomplish in reference to this goal.

My Personal Goals
After the above form is submitted, cut and paste your Content goals into My Personal Goals form.

To locate yr 3 yr Plan from last year.
Select “View My Portfolio for Full List” at bottom of Homepage
Locate Three Year Individual Educator Plan
Under Activity Info, click on Blue words: Three Year Individual Educator Plan

How to Copy Content Goals to My Personal Goals in MLP
After you complete your Plan for the year, you will need to copy your Personal Goals from your Plan into MLP for it to show up on all requests.
Locate Your Goals
- Under Approved and In Progress
- Click on Three-Year Individual Plan
- Click on Form Name in Blue at Top of Page
- Scroll Down to Part Two of this form where your goals are located
- Copy and Paste Your Goals to a Word or Pages Document

Creating Personal Goals
1. Click the My Personal Goals link on the left navigation bar~
2. Click on your Old Goals to make them InActive
Choose No after the word Active
3. Click the "Add New" button~Click the "Add New" button to add a new entry
4. Complete the Personal Goal form~
Complete the fields on the Personal Goals form.
NOTE: The Goal Title will appear in your Request Forms so make it relatively concise.
